CANCELLATION AND MISSED APPOINTMENT POLICY

1. Cancellation/No Show Policy for Dental Appointment:

At Craig Arive, DDS, your dental health is important to us, which is why we reserve time to serve your individualized needs. We strive to be on time for your appointments, and ask that you give us the same courtesy.

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment book.

We ask you, our patient, to give us a minimum of 24 hour advance notice for cancellations or to change your appointment. Please note, all Saturday appointments need to be changed by 12:00 pm on Wednesday and Monday appointments need to be changed by 9:OO am on Thursday, or a cancellation fee will apply.

Office appointments which are cancelled with less than 24 hours notification will be charged the following fees:
• 1st occasion – $50
• 2nd occasion – $50, plus a 50% non-refundable deposit on all upcoming treatment for 6 months
• 3rd occasion -$100 plus 100% non-refundable deposit on all upcoming treatment for 12 months
The Cancellation and No Show fees are the sole responsibility of the patient and must be paid in full before the patient’s next appointment. Please note that such fees will not be covered by your insurance.

2. Numerous NO SHOW Policy:
Patients who do not show up for their appointment without a call to cancel will be seen on a same-day basis only. Treatment will be required to be prepaid for all future dental appointments. All cancellation fees will apply.

Our practice firmly believes that a good doctor/patient relationship is based upon understanding and good communication. Questions regarding the Cancellation/Missed Appointment policy should be directed to our office staff.
